mySNC Forms: Complete Guide

Creating your form

  1. Manage your group
  2. Click on Surveys & Forms dropdown and select All Surveys & Forms
  3. Click Create

  1. Select what type of form you would like to create
    • Forms: Collect any data or documents from individual users
    • Surveys: Survey multiple users and review aggregate results
    • Polls: Ask a single question and view live results
    • Elections: Create positions and candidates and view voting results
    • Quizzes: Create questions and attribute points to correct answers
    • Jobs: Allow users to apply for jobs (if using the Career Module)

  1. Personalize your form to include a Title, Introduction, and Page Name

Question Types

mySNC Allows you to add the following question types:

    Question Type Icon Short Description
    Free Text (single-line) Uploaded Image (Thumbnail) Think of this as a short answer question
    Multi-Choice with Single Selection Uploaded Image (Thumbnail) A question where you want people to choose one option
    Multi-Choice with Multiple Selection Uploaded Image (Thumbnail) A question where people can select multiple option
    Multi-Choice with Dropdown Uploaded Image (Thumbnail) Like "Mult-Choice with Single Selection".  Users are expected to select one option
    Free Text Multi-Line Uploaded Image (Thumbnail) A question where you might expect the answer to occupy a few lines of text
    File Upload Uploaded Image (Thumbnail) A question where you expect to receive an uploaded document or image
    Date Selection with Calendar Uploaded Image (Thumbnail) Offers the form taker the ability to see a calendar and select a date
    Group List Dropdown Uploaded Image (Thumbnail) Allows for people to select from a list of mySNC Group types.  We're not sure where this would be used
    Ranking Options Uploaded Image (Thumbnail) Question type to gauge preference of given options
    Election

    A tool to facilitate officer elections
    Signature Uploaded Image (Thumbnail) Non-legally binding signature question type
    Quiz Uploaded Image (Thumbnail) A question with a correct answer
    User Selector Uploaded Image (Thumbnail) A question type that allows a person to search the SNC's directory for a "user".  We're not sure where this would be used
    Rubric Uploaded Image (Thumbnail) Allows a person to select from options with a title and description.
    Binding Fields (allowing you to pull fields from approval processes into the form as answers) Uploaded Image (Thumbnail) A variable enabled question type

     

     

 

 

Form Settings and Customizations

  • Basic Information:
    • See which group officer is the author for that form
    • Change the form name and type
    • Provide verbiage for a description and custom confirmation message
    • Connect your form to a workflow
    • And establish pass/fail quiz settings (if applicable)

  • Instructions:
    • Provide verbiage for a closing sentence as well as a message displayed on the submission confirmation page

  • Open/Close/Cap:
    • Set Yes or No to close the form or keep it open, respectively
    • Enter in a date and time for when the form should automatically open and/or close
    • Set a capacity for how many submissions are allowed for this form

    Access Rights:
    • Set Yes or No for if log-in is required, if answers should be anonymous, if submissions can be edited, and if users can submit the form only once
    • Select the group officers that should be the only ones to manage the form and view form results
    • Scroll down to Privacy and set the Access Rights to decide which users should have access to complete the form

  • Notifications:
    • Enter the officers who should be notified when a user has submitted a form, as well as those that should be notified when a submission has been approved or denied
    • Provide verbiage for a message that is sent when a submission is approved, denied, or put on hold

  • Approval:
    • Set Yes or No to hide the approval box or keep in shown, respectively

  • Field Binding:
    • Choose the specific approval processes that will allow you to select certain fields to pull into your form

Follow these steps to get started:

  1. Manage your group
  2. Select the Surveys & Forms dropdown and select All Surveys & Forms

  1. Create or Edit an existing Form

Question and Page Logic

  1. Be sure to Hide the page or questions that should be invoked by logic

  1. Click on the Logics button for the question that should invoke the hidden question or page to display

  1. Click Create Logic

  1. Set your Logic Type as Question or Page
  2. Set conditions in the If and Then sections
  • e.g. If the answer to “What year are you? is First-Year, then show the question “Please tell us about your involvement/extra-curricular experience before coming to college.”
  • If using Page logic to show hidden pages in your form, the final page of your form cannot be hidden and must be visible.

Workflow Logic

Workflow logic will allow you to invoke a step that is part of a workflow

  1. Navigate to the step of the workflow that should be invoked and be sure the step is Not activated by default so that this step only displays if the condition is met

  1. Navigate to your form and click on the Logics button for the question that should invoke the workflow step

  3. Set conditions in the If and Then sections
  4. Choose the Step of the Workflow that should be invoked based on this condition

Submission Tag Logic

Submission Tags allow you to automatically tag each submission so that you can easily filter by submissions that have certain tags associated with it

  1. Navigate to your form and click on the Logics button for the question that should automatically add the tag

  2. Set conditions in the If and Then sections
  3. Choose the Tag that should be applied based on this condition
